Available After Completing 90 Day Probationary Period:
♦ Insurance package - Our health insurance
carrier is Blue Cross/Blue Shield. There are two plans to choose from: PPO and a
High Deductible Health Plan with a Health Savings Account (HSA) (see costs
sheet). FCS pays the premium for your life insurance in the amount of one times
your annual salary (a minimum of $25,000 and a maximum of $50,000). Voluntary
short and long term disability, additional life insurance up to a maximum of
$350,000 with no medical questions asked (can request more but must be
approved after submitting a medical questionnaire), vision and dental insurance
and dependent coverage for health, vision,dental and life are available through
payroll deductions. Coverage begins the 1st day of the month
following 3 months of employment.

Paid Time Off Benefit
♦ Full-time employees start accruing Paid
Time Off (PTO) hours from the beginning of their employment with FCS. After
six months of employment, an employee will be eligible to use their accrued
PTO hours for any time off they need or wish to take. Beginning with the first
year and through the fifth year of employment, a full-time employee will accrue
18 PTO days. After 5 years of employment, the PTO accrual rate will equal 23
days and after 10 years of employment the PTO accrual rate will equal 28 days
per year.
Available After Completing One (1) Year of Employment
at FCS:
♦ Tuition Reimbursement Program for employees
wishing to become an LPN or obtain either an AS or BSN nursing degree.
The FCS 401(k) Plan:
♦ After you have been employed at least
one year, have worked at least 1000 hours, and you are at least 21 years of age
you may enroll in the 401(k) Plan on the first day of the next calendar quarter.
♦ You may contribute up to the maximum
amount allowed by law. FCS will match your contribution at 100% for the first 3%
of your pay that you contribute to the Plan, and then the match is 50% for
the 4th and 5th % of your pay that you contribute to the
Plan.
♦ You are 100% vested in the company’s
funds when the contribution is made to your account on a bi-weekly schedule.
